Overview

 

Pleasant View is a licensed skilled nursing facility located just behind the court square in the heart of Bolivar TN. The community is comprised of five Neighborhoods where up to 67 Elders can live: Love Boulevard, Hope Drive, Joy Lane, Faith Road and Peace Street. Here at Pleasant View our Elders have chosen to be referred to as Neighbors and they reside in either Private or Shared Apartment (Resident’s rooms).

 

Specialized Clinical Services

 

Long Term Care – For those neighbors who need a permanent placement. Pleasant View provides licensed nursing staff 24 hours a day, seven days a week and our direct care staff provides assistance with Activities of Daily Living.

 

Postacute Care – For those neighbors who need specialized medical and complex services. Our specialized services include:

 

•   Aggressive Wound Management

 

•   Tube Feeding and PEG Tube Management

 

•   Nutritional Enhancement Programs

 

•   Incontinence Management through facility services  and Contract with SilverCare        

     Solutions

 

•   Intravenous Therapy

 

•   Contracted services for Medication Management of our neighbors that are

     Memory Impaired.

 

Rehabilitation Care – Designed to restore our neighbors to the highest possible level of independent living. Pleasant View offers; Occupational Therapy, Physical Therapy and Speech Therapy.

 

Hospice Services – The facility contracts with several Local Hospice agencies who provide comfort measures for the neighbor and grief counseling services for the family members.
