Cornerstone provides elder care services ranging from post-acute care for those with complex medical conditions, to long-term care services for those who are not able to live alone and require medical supervision. We are committed to providing coordinated and solutions-oriented care in a “human habitat” environment.
Cornerstone Health Care provides distinctive levels of care including:
Post-acute Care is a cost-effective alternative to extended hospitalizations. This service is for elders whose medical needs require short-term, post-hospital care in a skilled nursing facility. The clinical outcomes for individuals at this level of care often allow patients to return home or to an assisted living center.
Long-term Care is provided for elders who can no longer live independently and require daily monitoring and management of a chronic illness or compromised medical conditions.
Assisted Living Care is provided for persons who do not need around-the-clock, nursing care but need assistance with their daily living activities.
Respite Care is offered for persons who need short-term care during the temporary absence of the elders' family or regular caregivers.
Adult Day Care is offered at some of our homes for individuals who may live in the community with family members but are unable to remain alone during the day. We offer a protective environment that includes meals and snacks, social activities, basic health monitoring and emergency nursing care.
Cornerstone offers various enhanced care services for elders requiring varying levels of health, wellness, and support programs.
These include the following:
Specialized Clinical Services are provided for post-acute and skilled care elders, includes physical, occupational and speech therapy, IV therapy, wound care, PICC lines, IV therapy and respiratory therapy.
Hospice Care is provided for individuals with a terminal illness that would benefit from pain management and emotional support services.
Bariatric Care is provided for individuals that are under a physician’s care for obesity and require long-term care.
Dementia Care is provided for cognitively impaired elders who need to live in a secure, safe and nurturing environment.
Behavioral Care is provided for individuals with behavioral problems that benefit from a structured social environment to encourage them to develop adaptive social behaviors.
