Overview

 

Milan Health Care is a SNF/ICF home where up to 66 Elders can live. It is located in Milan, TN and serves Elders from Gibson County and other Tennessee Counties. The population of those living at Milan Health Care is very diverse ranging from 34-94. Our home is a secure home with a large courtyard where Elders enjoy sitting on bright sunny days.

 

 

Specialized Clinical Services

 

Milan Health Care primarily serves those Elders struggling with behaviors related to dementia and mental illness. We have contracted Psych Nurse Practitioners available for medication management as well as a contracted Licensed Clinical Social Worker available for psychotherapy services. Physical, Occupational, and Speech Therapy Services are available to meet the rehabilitation needs of the Elder.

 

 

Community Involvement

 

Milan Health Care is involved with Tennessee Health Care Association and American Health Care Association, local area volunteer organizations and community events.

 

Innovative Approaches to Care

 

Milan Health Care has a program composed of a Social Worker and a Program Director who directs the Activity Program and Behavior Modification program at the home. Program techs work along side the Program Director and other Care Partners to meet the needs of a younger more energetic population. Our Elders enjoy monthly outings to the City Park, Restaurants, Baseball games, and local festivals. The It’s Never 2 Late Computer Technology system is also available for our Elders to use as well as Wii games to help meet their activity needs. Milan Health Care was featured in a local newspaper article last year giving credit to the Eden Alternative philosophy for the improvements in quality of care at Milan Health Care.
